A cunning, deliberate, underhanded situation in which supervisees are thrown under the bus by their inept, insecure, thoughtless, (and usually steatopygic*) supervisors, often in collusion with others.

Urban dictionary: “A situation that is totally screwed up, especially as a result of managerial incompetence.”

Originally of military origin;  meaning both multiple screwups, as used in the term “cluster bomb,” and referencing the oak leaf or star “cluster” insignia of the officer or officers who made the decision/plan, resulting in enlisted personnel being thrown to the wolves. Also known as “Charlie Foxtrot.”
